All athletes must complete clearance before participating in any team activity.

Step 1: Create/Log Into Account

• Visit athleticclearance.fhsaahome.org
• Choose Florida
• Select Bozeman High School
• Complete or update your profile

Step 2: Complete Required Forms

• Personal Information
• Medical History
• Parent/Guardian Consent
• Upload FHSAA Physical Form (EL2)
• Sign all required electronic documents

Step 3: Get a Sports Physical

• Use the official FHSAA EL2 form

Step 4: Await Approval

• Watch for an approval email from Athletic Clearance
• If incomplete, you’ll be contacted to resolve missing items
